I first access past incidents, events and cause. You must first get a baseline of areas of risk by historical events. The next step is to look at current policies in order to learn where or what caused the incident. Where did our process fail. Typically most accidents and injuries have more than one cause. From there we can look for ways to eliminate or reduce the risk moving forward. T this point we must train our staff of the changes. (Short version).

See comments above: In my experience the steps i outlined ( though a more thorough account and steps ) have always proven to reduce accidents, develop safer policies and procedures and reduce workers stress regarding safety.

Effectiveness of a PSM can be measured in how it helps in the overall PSM Goals. One needs to define and set in motion the various activities needed for a PSM program goals. For activity measure, we need to collect feedback how we want things done and how things are being done- that is basically what auditing does. This though is not enough. A learning from incidents and near miss program where contribution of each PSM activity to incidents are throughly analysed and corrected and activities strengthed needed to achieve PSM Goals. Feedback from various stakeholders; operations, maintenance, management, etc on how they see the contribution of the various activities has been also a good measure of PSM effectiveness

I feel it’s important to learn from historical data involving both leading and lagging date. Audits, inspections, employee feedback and historical data. Depending on the results of collected data, along with a represented groups I can now revise current PSM’ and PHA’s implementing the information. In all situations I develop JHA’s for all definable features of work,. I introduce the requirements and steps used to create JHA’s and PTP’s. It’s important to next do a limited rollout to a select group. Throughout this rollout I review the submitted JHA’s for completeness and correctness of the information. I ask each member of the test group to make comments both positive and negative. I provide constructive feed back. Once approved, train

I agree with above as a good place to start. Without having and reviewing information, you don’t have a valid starting point. You wouldn’t know where to problems are if there are problems. While it’s important to review data, it’s additionally important to score risks in order to assess which policies or procedures should be addressed first.

I again agree with the above, but, I think you must know the goals, what the stakeholders want to see, at the same time as the collection. The company could be planning a new business model, they may require new and improved PHA’ and PSM’s due to new chemicals, and they require procedures from the start . New or changed regulations may have been implemented. It is just as important to know why, as well as what data you will need to reach the desired out come. You don’t want to waste time and efforts in data collections that has no relevance to the end goal.
