Actually, a HAZOPS (the ’S’ is for Study) is best used for process or material flow risk assessments - so the machinery would be included in the material flow. Identifying hazards is best done with the widest group possible, but especially with those using the tools and equipment, and they must be allowed the time to think about the hazards. I have found it especially useful to have the maufacturer present to help with risk assessments, at any time they are willing to attend, even after the tools and machinery are in use. Risk assessment is an iterative process, remember to schedule an update, and soon.

In my opinion the first step in managing tool and equipment hazards should be in selecting the appropriate equipment based on the intended use, who will be expected to operate the tools and equipment, any user training and guidance from manufacturer prior to installation or delivery and working through a theoretical risk assessment process prior to purchase. If the tool and equipment presents additional risks to people, plant or property and there is an alternative process or newer technology that eliminates or substitutes this risk by automation or reduced exposure to hazards at an acceptable cost, this option should be explored.

An example way I'v seen elimination of hazards in workplace is establishing 5S requirments. 1.Sort is sorting through all items in a location and removing all unnecessary items from the location. 2.Set in order is putting all necessary items in the optimal place for fulfilling their function in the workplace. 3.Shine is sweeping or cleaning and inspecting the workplace,tools and machinery on a regular basis. 4.Standardize is to standardize the processes used to sort, order and clean the workplace. 5.Self-discipline or Sustain is the developed processes by self-discipline of workers. Its a matter of pride between colleagues that their equipment is ‘proper and correct’ - so encourage them to mark each other for maintenance and inspection routines. Prohibited items should be included, including homemade tools. Availability of the correct tools for the tasks should also be included in inspection routines - if I have to walk the site to get the correct tool, I may self-optimise and use another incorrect tool to get it done - unwittingly the employer will have encouraged the wring behaviour.

This is perfect in theory. The reality is often quite different. As an example, motor vehicle technicians work with lifting equipment such as ramps and lifts daily. They are inspected 6 monthly by insurance engineers and often maintained either by planned preventative maintenance or as a result of the insurance engineers report (reactive maintenance). There is an expectation that technicians should routinely carry out regular inspections of lifting equipment and we, as safety practitioners, encourage this. However, on occasion we are asked “what qualifies me to say if this equipment is safe and operational?”. Simplifying the process by implementing a daily “dry run”, operating the equipment to see, hear, feel if anything is different.

It’s always a good idea to be our brothers keeper. Don’t be afraid to stop someone using a tool incorrectly or one that is defective or pointing out a potential hazard around them. We don’t have to be rude about it but I would feel better in stopping them from an injury rather than visiting someone in the emergency room or hospital.

Changes to personnel, changing worktime patterns, temporary external maintenance people coming on site and other works in the same location - these all contribute to how a person works.
