In my opinion, one need to set clear standards, use equipment that's been checked and calibrated, and train the people doing the tests. (Very Important) Also... test some known samples to make sure the tests are working correctly, and keep the testing area stable. Review and update the tests regularly, use statistics to monitor the process.…and keep track of all the results and samples. By doing these things, you can be sure that your quality control tests are giving you accurate and helpful information.??

You need to consider that there will always be edge cases, a deadline that will not be met or a miscommunication between QA and Dev teams. Some features are not straight forward, others just clutter three flow that the user is already familiar with. The QA team should always give feedback regarding new features and how easy they are to grasp.

Defining correct test method , acceptance criteria are key elements. Maintaining valid calibration and competent staff is essential. Result shall be signed off by authorised , who can only have access to do editorial corrections. Remember test results are records and shall not be edited to suit your needs.
