We need to be braver at identifying what went wrong during implementation (while these issues are invariably dealt with through iterative processes that get the project back on track, there are often mistakes or unanticipated challenges that could be used to build a solid, searchable knowledge bank for the organisation that doesn't depend on individuals recalling their involvement in different programs). This will add value & enhance efficiencies for future programs.
