I’m a psychiatric nurse practitioner I advocate for my patients every day I feel we could do so much better, but we continue to except the mediocre. I see my colleagues in healthcare, and they also reiterate what I am saying things have changed in healthcare over the past 30 years, but not for the better.

I couldn’t agree more with the importance of identifying stakeholders' perspectives, but I believe this step should come first, not fourth. Gaining buy-in from leadership and the team members who will use the new innovation is crucial to its success. All too often, we implement changes based on textbook solutions without considering the full impact—on patient outcomes, clinic workflows, financials, and alignment with the organization’s mission. Starting with stakeholders’ input allows every part of the solution to align with the broader goals, paving the way for a more seamless and impactful transformation. When we prioritize their perspectives from the beginning, we set the foundation for changes that truly benefit everyone involved.

The fifth step is to consider the context and generalizability of the impact of the health care innovation. It should evaluate both the potential benefits and risks of healthcare innovation. Additionally, ethical principles such as respect for patient autonomy, beneficence, non-maleficence, and justice should guide the development, implementation, and evaluation of healthcare innovations. The involvement of healthcare professionals, patients, policymakers, and other stakeholders is crucial to ensuring that healthcare innovation adheres to ethical values.
