The monitoring of the SOPs for gaps is critical, but when a gap is identify it is just as important to fill it correctly. Far too often I have seen rushed corrective and preventative actions (CAPAs) that fix the gap in the moment but do not address the root of the problems. It is critical to take into account the perspective of the boots on the ground and to follow up critical gaps with an effectiveness check to ensure smooth operation in the future.
