In my experience this is a very important aspect of this subject. Actually not only in this subject but for any thing that you want to start or implement. Without clear goals and objectives, as well as a methodology to measure the success of initiatives that lead to these objectives it will end up with uncertainty and undesirable results.

What I see far to often, that surveys are created for things I do not need to ask, because I know it already. Or questions are made to individuals, that do not have the insights to make a good assessment. Sometimes big numbers do not tell any insights at all and not each anecdote helps you to get a better understanding. Survey and sample design needs profound knowledge and someone who challenges your approach.

Open-ended questions are good for interviews; however, they should be used with care in surveys. Evaluating responses from open-ended questions cannot be automated, and you usually lack the opportunity to ask follow-up questions, which are often necessary. In that respect, offering unbiased and complete options for single- or multiple-choice questions is essential. Unbiased means you should not, knowingly or unknowingly, steer the response in a predefined direction. Complete means you will only get value from the survey if you give a full range of answering options, including an 'other' category to cover unthought cases.

Start by accepting that generally customers don’t want to complete surveys and you are likely to get a low response rate. Pay more attention to the data you already have access to. Customers are continually providing feedback on products and experiences. Form a hypothesis, understand what data you need to test the hypothesis and check what data you already have access to. You probably have more evidence than you think.
