Detailed documentation is necessary and even more important is presence of the stakeholders responsible for carrying out updates, as they are well versed with the procedure and have been doing it all their lives, so they must be on site preferably whenever any big update is being pushed. If possible DR infrastructure be kept hot standby to overcome any crisis situation

What is most important in execution of the update is stable internet connection or uncorrupted checked media (CD/DVD/HDD/image) why i am saying so that i have seen many times despite best arrangements and all checks in the network part, the internet connectivity through which the update was to be done fails and there is no back up. similarly sometimes there is a single DVD image which was to be used but it turns out to be non functional. So double check your media when you execute your update including power arrangements for bigger updates
