"Life is an exercise in risk management" is a great way to look at this. Looking back at my life, and looking around at other people's life, I think that the more explicit we are in articulating and prioritizing these risks, the better decisions we can make. Sure, we will never have enough data to make a perfect decision, but at least if/when something blows up we can rest assured that we did the best we could with the data that we had. Not good to second guess ourselves. Just take a risk management approach, make a decision, act on it and move on. Revisit and pivot if necessary in the future, but avoid "analysis paralysis". Use any of the best practice frameworks to identify and prioritize risk, just be intentional and explicit.

A sometimes overlooked aspect of risk-based testing is to apply testing resources proportional to the risk at hand. Too often I see testing teams having fun spending time on edge cases that are esoteric. Yes these edge case scenarios can happen but any hour spent on these edge cases are hours that are not spent in maybe less fun, maybe more mundane, but certainly more likely and risky scenarios. Why do we know that they are more risky? Because we have taken a risk-based testing approach...

Adopting a risk-aware culture, and beyond that adopting a risk-management approach to QA is not that hard, especially when one realizes that there are only a few things we can do with risk. We can (1) accept it, do nothing and continue business as usual, (2) refuse it, veto the whole initiative and move onto other things, or most probably (3) mitigate it. Mitigate is the grey zone between accepting and refusing risk where we accept the risk provided that we do stuff to minimize risk and bring it to an acceptable level -- for example QA the hell out of it... BTW -- transferring risk is a form of mitigation...

This is key, and something that I see too many organizations do a poor job at. Learning from risk is not only about someone getting tribal knowledge about something that happened, it is about knowledge management. Which means, a formal process with tools to support this initiative.
