How can you quickly identify the root cause of a quality issue?

Collaborating with the interdisciplinary team to write a clear problem statement is the first step in identifying the root cause. Asking what the problem is and why it is a problem is key. Then, you can use the 5-Whys tool to drill down to the actual cause instead of surface level issues. There should also be a focus on system problems vs. people problems.

RCA can help you prevent recurrence, improve customer satisfaction, reduce costs, and enhance performance. I usually start from reports as an example like failed test reports, usually you will find the mistakes on the same report and so you save time and effort. Secondly try to filter the sources of variables to minimize the possible root cause for verification, I do this by filtering details such; date, time, location batch, type or group and so on untill you come to the most signigicant and mostlikely the exact root cause.

Data certainly plays an important role in analysing a problem by trending, stacking & grouping using various statistical tools and plotting then in different graphs- bar, line, spider and many others depending upon the requirements.

All data relating to operations need to be calculated and stored using data loggers and fed through data model systems to different applications. A more advanced way to deploy the data collection activities would be to use Industry 4.0 and smart manufacturing guidelines to extract and cognitively extract inferences from sensors and actuators in the most comprehensive way.

Root cause analysis is fundamental to solving many problems in many areas of work including Occupational Health and Safety. This technique is very valuable to take an organization to the next level in its operations enhancing its effectiveness. As all valuable tools there is a need for a mature organizational culture to utilise it. In many of my experiences I have seen very superficial RCA's just made to be put in a file and to be shown to the auditors with no effect at all. Techniques are vital but the foundation of culture must be laid first. This is true for all companies small or large.

I think this is one of the most important steps of problem solving. If the hypothesis is not being tested, the efforts and resources applied to implement actions for what we believe is the root cause might be a waste of time and of money. There will be times when you can't replicate the failure mode as it would be very expensive, it was an isolated event or the way to replicate it is by a destructive test. In those situations I would recommend the use of one of my favorite tools for prevention: FMEAs. With a FMEA you can analyze what potential causes would have a similar effect and also you can prioritize which actions to implement by the risk priority number.

Once you've identified a potential root cause, rigorously validate it through controlled testing and data analysis. Implement changes or countermeasures to confirm if eliminating the suspected cause resolves the issue. Utilize statistical tools like control charts or FMEA to assess the impact of these actions and weigh the risks and benefits, ensuring a comprehensive and validated solution to the quality problem.

Prior to implementing solutions, the frontline staff should weigh in to see if the proposed solution is viable. Often times, they may actually have a better solution than leadership. There should also be a pilot completed to implement a proposed solution on a smaller scale and see what works and what doesn't and make rapid cycle changes.

Ensure a comprehensive implementation plan is in place, clearly outlining the steps, responsibilities, and timelines for executing the solutions. Document the entire process, capturing key insights and lessons learned for future reference. Continuously monitor the impact of your implemented solutions using relevant KPIs and feedback mechanisms, enabling you to make timely adjustments and sustainably improve both quality performance and customer satisfaction.

I would suggest that quickly identifying the root cause of a quality problem is possibly not the correct mindset to have as if your main goal is to quickly identify, you could be pushed into identifying the easy to find root cause and not necessarily the correct root cause, you might only identify an underlying root cause and leave the main root cause to go unidentified. Also do not go into a root cause analysis with your mind already made up of what the cause was as you could quite possibly overlook other causes, especially if you are leading the investigation.
