Risks can be identified through the design & development process, design reviews, through customer feedback and complaints, post market surveillance, literature review. In my experience, I find the dfmea very effective in identifying risks, use the dfmea in combination with a control plan to assess the risks.

Select the right Tool for the job. Depending on your situation, there are various quality tools available. Example, PESTLE, 5 Whys, SWOT, Kaizen, 5S, Pareto, Poke-a-Yoke. Having the right tools will help solve the problem, using the wrong tools may not get to the root cause of the problem. Always perform effectiveness checks!
