How can you measure the impact of improved physician communication on patient outcomes?

First and foremost, we as medical professionals need to understand that communication is a dialogue and not a monologue. As simple as that may sound, we are often cought up in our own jargon and fail to appropriately gauge the level of understanding of the patient or listener. Therefore, imperative in assessing our communication skills as physicians is our ability to effectively connect with the patient. Surprisingly, it is not only by words, it can also be culturally and sometimes emotionally.

As long as we continue functioning in a fee for service environment, and wRVUs drive compensation, providers are not afforded the time to have meaningful engagement with their patients. Maybe a new role has to evolve where a patient has a full time “liaison” that maintains an open line of communication with full access to medical history, medications and appointments.

We spend time everyday to train our fellows to improve communication skills: synthesis, appropriate wording and body language to transmit a sense of reliability. It is a journey.

Considering that physician-patient communication is difficult to assess quantitatively, a qualitative approach is required. One way of achieving this is to introduce Patient Reported Experience Measures (PREMs) into the the pathway

In my experience, communication has to be “effective” for it to be meaningful to the listener and some time the sender of the message must learn to be an active listener some times. For Physicians communicate effectively and achieve the desired patient outcome, they have to be active listeners as well. Feedback from our patients, their care givers, colleagues and peers is an effective way to assess our communication skills. Effective communication would mean fewer unexpected and unwanted patient events, more patient compliance with care.

Courses are important. But more important is to understand that today teamwork is key and miscommunication is malpractice. Therefore focus on each individual word and keep it simple and clear. Avoid confusion and ask for immediate feedback from the team members.

One thing I've found particularly helpful for communication skill improvement is in person peer role-playing activities. This creates an environment with a clearly stated goal of assessing and improving communication skills. While it may feel uncomfortable, receiving immediate actionable feedback on patient communication is invaluable.

In order to adequately show improvement you have to identify the areas of concern. What outcome issues seem to be tied to communication and how that might be addressed. I think that it is always helpful to see if patients report they feel as if they are listened to by physicians and given adequate time to discuss issues with physicians. I believe I was once taught “ if you listen to the patient they will tell you what is wrong with them”. Again, what is the problem and how are we going to address it and measure it needs to be included in planning of any communication skills education.

Measuring the impact of communication skills on patient outcomes is challenging. Many patient satisfaction surveys inquire about general healthcare experiences as opposed to specific provider encounters. It is also not possible to assign cause and effect looking solely at retrospective data, and controlling for the impact of communication quality further confounds the results. I believe we need better standardized tools for directly measuring the impact that individual provider communications have on patient outcomes.

It is essential to recognize that improved physician communication not only influences patient outcomes but also has a profound impact on the physician's own professional fulfillment. Physicians who can establish meaningful connections with their patients often experience increased job satisfaction and reduced burnout rates. The ability to communicate effectively with patients enhances the sense of purpose and fulfillment that many physicians seek in their careers.

I believe this is the most important parameter to measure. We are practicing a patient-cantered care medicine, and any possible outcome should be primarily measured by patient themselves. The process starts by surveying patients to get their perspective in regards of how their doctors communicated with them and what could be done to improve the at. The following step will be measuring the outcome after implementing patient’s suggestions into the training modules provided for staff.

Dr. Smith, a cardiologist, implemented a feedback system where patients could anonymously rate and comment on their interactions post-consultation. Over time, Dr. Smith noticed recurring feedback about medical jargon confusing patients. Acting on this, he began simplifying explanations and using visual aids. It involves contemplating one's experiences, feelings, thoughts, and actions, with the intention of learning from them. When applied to the realm of physician-patient communication, reflection becomes a key to unlocking the door to enhanced patient outcomes

Every acquired skill should be followed by a reflective review to understand it more deeply. Examine closely to identify mistakes, potential modifications, and elements that can be eliminated. Recognize the areas where we excel, especially in challenging domains like communication. Acknowledge the skills we learn and strategize on improving areas that require further development.

Measuring the impact of improved physician communication on patient outcomes can be exemplified by the Cleveland Clinic's "Communicate with H.E.A.R.T." program. They trained staff in effective communication and empathy. Post-implementation, patient satisfaction scores, measured via the Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS), saw a significant rise, especially in areas related to doctor-patient communication. Additionally, there was a decrease in patient readmissions, indicating better understanding of post-discharge care. Effective Physician communication between patients and colleagues can be hard to measure. It's not something most medical schools emphasize. But, in terms of patient outcomes, I'll argue it might be the differentiating factor between good and bad providers. If patients feel communication is poor, they will not follow through with the prescribed treatment plan.

This information is somewhat echoed in previous paragraphs of information. All health care providers and professionals should be trained in evidence-based communication tools agreed upon by the Interprofessional team. For example, if SBAR will be used, educating the team on SBAR and its use will support the team in understanding that they will be expected to communicate objective information in a succinct manner. As a receiving team, I would also know to actively listened and wait for the R - recommendation before asking questions.
