To echo what Arnold Remal said: make sure the activities match what the goal is. Don't just have a "team building" activity of playing a game, just because it sounds like fun. I have felt success when people have pointed to an activity that we did and made comparisons to a job they were currently working on. Adult learners want to feel like they are getting closer to their goals, not that they are having their time wasted by figuring out what color M&M they are.

I think this is where a lot of organizations struggle to show the progress that they are making. Have the survey, but then show exactly what is being done to build on those survey results. Another suggestion is to show the changes and progress being made on goals long before another survey goes out. It looks really desperate when a survey is sent out and then you see a "just as a reminder, this is what we've been doing to show we are listening". Results shouldn't have strings attached.
