One of the challenges during problem definition is to confuse symptoms with the root cause, hence teams focus on addressing the underlying issues but not what really causes the problem. A good practice during this phase is to ask "why" multiple times until we exhaust all potential causes. Adding the goals and benefits of the project would also help to get buy in from stakeholders.

This step is key in the process one step is extremely important is not only listen the process reviewing KPI’s but also listen the employees feedback and perspective. This will allow to understand if they know the customer feedback and if the really understand customer requirements as well as to know chronic problems reported by employees but oversee by the administration.

I brought DMAIC from my former semiconductor to new product development, as well as, troubleshooting quality issues. Of all the steps - Defining the problem is the most important because it aims the team in the direction of root cause exploration. Measurement tells you what's out if spec and what the desired state is. After this an Ishikawa is what we typically used with as many different insights to the problem as we could get nearly every group participated w representation. After we had highlighted and voted for our top three, a design of experiments and necessary equipment was gathered. Solid documentation was required as well. I can honestly say of the numerous times this was done - we had a 100% success rate.
