The best process I've used is having an employee classification system. Certain classes of employees within a department that have achieved specific skills are paid a wage range. If an employee achieves the necessary skills to advance to the next level they are promoted to that level and receive a wage rate within the new classification. Example: Sara is an entry level Patient Services Representative (PS1). After several months she has mastered the Hygiene/Recall system which qualifies her for the PS2 classification. She is notified of her promotion and her wage is adjusted to the lower portion of that classification. She is in essence rewarded twice by receiving a raise and a promotion.
