How can you ensure HIT systems comply with interoperability standards?

There're many definitions of healthcare interoperability out there. HIMSS has their 4, where on top of the aforementioned there's an organizational layer. There're 6 layers suggested by the European Commission under its European Interoperability Framework that adds more on top such as legal and governance. Standardization efforts still mostly stay on the technical side defining data elements, formats, coding systems. The top layers that enable true interoperability are difficult to standardize. There're efforts such as European Health Data Space regulation which suggest aligned policy and governance, Time will tell how well those could be adopted.

Implementing interoperability standards revolutionized our healthcare delivery. Foundational standards secured data transit, while structural standards ensured data uniformity across systems. Semantic standards maintained data meaning during exchanges, translating to accurate, timely patient care. This systemic alignment fortified our HIT infrastructure and propelled our healthcare services forward, exemplifying a sound digital health ecosystem. Semantic part of interoperability is crucial and perhaps the most important. I'd go as far as to say if semantics of messages aren't ironed out and exchanging systems don't know what the other system 'means' with a message, it'd be way better if the exchange wasn't happening in the first place. Too many times we see integrations happening on a technical level and calling it a success, but then on the content level, it's a mess and can have serious clinical consequences.

modern architectures can leverage their potential only if data from heterogenous sources can be consumed, processed and served if machine readable formats like JSON come into play and a standard framework like FIHR sits on top. In a perfect world legacy systems can mapped to actual data standards.

The same word can express different meanings, depending on the context. An example would be the concept of headache which can be understood as symptom for a disease, an indication or a side effect of a treatment.

There are a number of different initiatives globally such as Global Digital Health Partnership (their report o interoperability advancements is worth of reading) and regionally. Some regions demonstrate considerable success, such as Gulf Cooperation Countries, where e.g. UAE or Saudi Arabia matured their domestic interoperability and show case maturity on syntactic, semantic and organizational levels and now they head towards establishing rules for interoperable HIE across the countries. European Union, even though doesn't similar levels on interoperability across the Member States, is pushing towards harmonized legislation, among many objectives, giving access to market to interoperable HIT systems.

you start with interfaces, which make data machine readable, can safely detect different context, identities and can decode and verify the authenticity. These interfaces must be available, documented and applicable to all sub systems based on REST standards or JSON formats.

identity, authenticity and integrity over long times depend on interaction with common systems like IAM (identity and access management) and a system of unique identifiers which can be validated and verified after long periods of time. Time frames must be defined by retention policies applied to the data to properly apply storage, archiving and controlled deletion of data.
