It is so important to do something with the feedback. You should let the leaver know if you intend to do, as vague as “feeding back a summary” can be helpful so you can decide the right action after the interview. If it feels complex I often share the few lines I’ve written as notes as I go to be sure they are accurate and it représentes them. Depending on the circumstances it maybe a short summary to the manager cc: mgr+1, it could be nothing new was learnt as the leaver said the same to the manager. Rarely this may need to be escalated so appropriate action can be taken. You maybe advising training, a grievance etc. If you’re doing a lot of exits, review them together looking for patterns.

Listen carefully, don't try to fight back, and prove the leaver that they are wrong, and you are right. At this stage it's not about the company, or yourself as a manager, it is about collecting some valuable inputs, putting them in context and understanding some trends, issues, struggles etc. The ultimate goal of such a conversation is to reflect and act at times on certain internal bugs, not emotionally harass the person who is leaving.

In my experience sending the questions along with the interview invite can be helpful. Not everyone will want to pre-read but gives those who do the time. I’ve seen it that the questions are sent and they need to reply in advance with statements and rankings. This works great for ranking reasons for leaving/ culture giving quantitive data but I’m not so keen for the qualitative answers as it can close off the conversation in real time. As others have stated, be kind, give empathy. I t can be difficult hearing some leavers so give your self support if you need it.
