When doing the backlog grooming, even before the start of the sprint, small teams can review the future tickets and comment on the ticket about any blockers, dependencies and grey areas you may have. This way it will be more productive during the sprint planning and will have clear goals and scope by the end of meeting.

I usually steer clear of putting too much emphasis on rigid goals and scope, as it can unintentionally discourage change. It's vital to create an environment where both the end-user and the team feel psychologically safe to switch gears if needed. That's why, in my experience, we often prefer discussing "user stories" over strict "requirements." Having said that, it is also important to allow the Team to focus on what they are working and limiting work in progress. Sit with a colleague and understand the product implementation, do a pair programming session, this way it would help you to identify business flows. Then create a mind map with testing scenarios integrating with test design techniques, check with other devs about what tests are covered in unit tests. Test the features early by setting up local machine or in dev environment. I think it is all about collaboration and being proactive and taking initiatives in this modern era of testing

Always aim for continuous improvement. The more you strive towards improvement the better your quality. The fact is no one can get it right in the first attempt is the thing that needs to be taken into account.
