Kind of feeling that #5 and #2 goes hand in hand. The biggest communication barrier with cross-functional teams is how one needs to communicate with everyone in the audience. It may often require using a mix of communication styles/ways so that the message be reached to a wider audience. Here is where tools can be a big help. The tool not only needs to be versatile but also need to be easier to learn and adapt so that it can be the norm.

The specifics around co-ordination such establishing a norm or cadence is very subjective depending on the size, operational complexity and how closely do they need to work together.
