Here's how you can navigate common mistakes when responding to failure in aviation.

Mistakes happen. It's not the mistake that usually causes an emergency, but failure to acknowledge the mistake. Overcoming this requires trust. Trust between crew and trust between employee and employer. Trust that admitting a mistake will not cause an unjust response but a response of, 'Thank you for letting us know, we will have look and see how this mistake occurred and see if we can prevent it from happening again.' Reporting mistakes should be seen as opportunities to improve and grow, not as failures.

When you make a Mistake in Aviation, Acknowledge and report it, So we have fewer Accidents. The cockpit should have only an operating crew, no observers because in case of emergency, observers might come in your way, suggest a stylized Cockpit.

Mistakes aren't the big problem, but, yes, the violation... ...you know that's not permited, or it's prohibited, and you do anyway...

Errors are to happen any time, humans are not perfect and it’s in its nature to make mistakes and learning from those errors, even machines are liable to errors, it is how you apply what you learned from that error to try and avoid it or how to handle it if it happens again Recognizing an error can be hard since you should accept if that error was your fault or not and based on your knowledge and experience the way you will handle it.

Being a human will lift the flag to keep your mine aware that you may forget to do or check something. If you recognize or realize it; you MUST take an action (no matter if delays your process for one minute) you better give an excuse, then not be able to tell the story ;)

Overcoming the limbic system response is a matter of training and exposure to high stress events. It is easy to say 'stay calm' but not so easy to actually stay calm. The usual stress response will generally cause focus on the problem, leaving very little cognitive capacity to formulate a solution. Being able to stay calm is a learned response, not a natural one.

Personality will influence a lot in how you can maintain that calm, you could be calm but might want to rush things or you could be so calm that it might take too much time. Proper training and good guidance from instructors helps on how to maintain a proper level of calmness.

Always breath and search in your brain for the right solution, NOT THE PANIC quicker action, it will lead you to make a worst mistake.

Human nature is not to stay calm, we go back to our prehistoric times with the fight, flight or freeze syndrome. In the flight deck, fleeing is not an option, so it’s either fight, which is quickly handling, but may cause more problems, or freeze. To overcome this instinct we have many tools or more specifically structures. First is Fly, Navigate, Communicate. Answering the simple question, is the airplane safe and is it going to be safe over the next period of time. Then it’s time for checklist work, if needed. To finish off we use a failure management model like (T-)DODAR, SPORDEC, FORDEC. These structures help us to ‘stay calm’.

Indeed, checklists are pilot’s best friend but it can also be your worst enemy if you don’t comprehend and understand what you are reading! It is important to read them in a way that both reader and listener understand and comprehend what the checklist demands, understanding the aircrafts systems is essential for a good comprehension and development of checklist’s, calmness while reading is also important, it is not how fast you can read it but how good you read it!

I always think of checklists that they are written in blood, meaning: someone paid with their life for this item to be where it is on the checklist. In a way this is true, there are items on our checklists that could seriously harm us or people around the aeroplane. Some other items are less dramatic. In real life we fly the airplanes using flows from memory, then back our actions up by doing the checklist. The checklist only contains the real important safety related items, otherwise checklists would be extremely long. The biggest question we need to ask ourselves is why is that item on the checklist. If we know that, then we know the importance of checklists.

Rules has being created based on other mistake, don't miss the opportunity to avoid becoming part of the statistics of Accidents. Take your time and do your checklist. Don't PROCASTINATE...

Training is the best foundation you can have, understanding it and continuously training will make you trust your learnings when the time comes and for those issues not written it will come in handy. Learn to leave no gaps, keep learning and ask on your trainings any doubts you have.

Repetition, Recurrency, Follow rules, cross Check, learn how to see the rules as your survivor best friend, instead of your must do list. Keep training will make you recognize the error faster then others. Learn how to fall in love with Aviation

This is a good one and the hardest one to assure 100%. Transmitter and receptor have to be in the same frequency in order to establish a good communication, differences in language pronunciation is also a factor even if speaking in the same language. Here Central and South America even though Spanish is the main language there are so many differences in aeronautical phraseology, interpretation of procedures or pronunciation of waypoints or fixes(especially those with funny names). Inside the flight deck, the first contact that the crew has between them is essential as to how your communication is going to develop during the flight, barriers should not exist and there should be good confidence between both pilots to communicate freely.

Having all experience may not provide you the MASTER Key... Accept others opinions there is always someone with a fresher mind that can offer a new fresh Idea. Knowledge will allow you to be an effective communicator. In an emergency, remember the message must be sent in a clear and short way.

In aviation, accidents are usually highly visible, and as a result aviation has developed standardised methods of investigating, documenting, and disseminating errors and their lessons Although operating theatres are not cockpits, medicine could learn from aviation. Observation of flights in operation has identified failures of compliance, communication, procedures, proficiency, and decision making in contributing to error. Accepting the inevitability of error and the importance of reliable data on error and its management will allow systematic efforts to reduce the frequency and severity of adverse events.

As part of the preflight briefing I always say that it is not a mistake until both of us make it. This fosters an atmosphere of active listening and monitoring. Finally, it is not who is right but what is right
