An example that works best for us is always speaking with the client before the event. They choose within their organization who asks questions. We are fully aware of who this person is and they have a conversation, again, prior to the event with the speaker for a win-win situation.

The movies "Notting Hill" and "Buckaroo Banzai" are great examples of how Q&A sessions can go out of bounds. "West Wing" TV show had examples of Q&A sessions or "pressers" being redirected from out of bounds. Study the history of press sessions and interview processes to see how cultures vary in handling Q&A.

Great public speakers are not just confident in making speech but are also adept at being fluid and adept in making smooth transitioning during live Q&A sessions. I thing I've noticed that they do is using coded language that the event is bound to take cognizance but actually instructs his co-host or his support team to take certain actions.

If there is likelihood of total loss of control, do not do it. Q&A assumes free flow and opportunity to redirect where the audience needs to hear solutions, understanding, or information. Play freely. As you freely received attention, freely give it. Ask for clarifications if the expression is not clear and easy to answer. If an audience member is unhinged or angry, help them. Give a little and you will get a lot.

If there is a question asked that will disrupt the flow of the Q&A, acknowledge it and in an excited manner and invite the individual to speak with you briefly after the talk if time permits. However, do your best to acknowledge it so that everyone feels inclusive when possible.
