Understand the Root Cause: Conduct a detailed analysis to identify what went wrong and why. Learn from the Experience: Encourage a reflective mindset among your team. Discuss what could have been done differently and what lessons can be applied to future projects. When faced with a project or system failure, honesty and transparency are crucial. The initial step involves acknowledging the situation without glossing over it. I was blessed to handle one such incident long back solved it (at a price). The customer started complaining that the application hosted in our premises was going down every morning. I just gathered my team to assess the RCA and encouraged them to be little more assertive to work 24x7 until it is fixed (short period only)- result- Success at a aprice of Compensating to the client's retention. Out come- 48 hours of sleeplessness, engaging the team members effectively, active support from the stakeholders. Finally- special team outing to celebrate the success.

Later as a formality we filled in the RCA from how we recollected every moments- I would say we applied Ishikawa and Cause- effect and ofcourse delphi methods. The main issue was every evening between 8 and 9 the all the server services were vanishing retaining only the server (Win 2008) services. this pattern when for 2 to 3 days until we observed it with the help of external security specialist. During those times the Infosec was in an infant state. Only larger orgs could affort the state-of the art security systems. THANKS TO ITSM AND ISO 270001 processes implemented later

In the IT industry, what I often refer to as an autopsy report is more commonly known as a Root Cause Analysis (RCA). There are 2 ways that we can plan strategically- One way- get the finger or hands burnt and later apply for preventions ( more prominently experieced by many of leaders and managers like me yet undisclosed for the sake of Facade during the interviews :) ) Other way- Learn by other's experience- either attend workshops or hire an implementation consultant. Over this years quite often to face system failure due to minimum budget, miss-behavior, lack of experience, careless from parties. My thoughts would be fix, find root caused, assess and monitor it closely. Along with many of requirements that coming in due to growth of business mistakes can't be avoid, we need to be more strategic in decide what action we need to do to face system failure and act quickly to adapt and to mitigate it
