Shut the work down until it is done to policy or terms outlined so that it is safe. I had contractors on a project that dropped some equipment and didn't report it to facility owners. Next morning, I contacted the rep and explained what happened and how it won't happen again and that we wouldn't restart the work until we got their approval. They approved it but my contractors were petrified OSHA would show up in mean time. You have to be willing to shut work down or some contractors will cut corners whenever your back is turned.

Every single personel under the project being the client or contractor must understand that a safe project ensures successful projects. Apart from the normal daily tool box talks and PPE inspection, the client should ensure a timely (and ad hoc) safety audit for both for documentation and site conditions. Strict actions should be taken to the contractor personnel such as reprimand from site for number of days and warnings to the contractor whereby if they repeat for x times, progress payment will be held as a penalty.

Before the start of project, have a detail meeting with the project officers which you assigned for different phase and it should include on design, technical, engineering portion that has been spelled out in contract Have a regular inspection on the site.
