I agree. After all risks are assessed, a determination should be made regarding the possibility of eliminating each one. If risk elimination is not possible, appropriate safeguards should be identified and implemented to prevent undesired initiating events and/or mitigate potential consequences. Once that is done, we can plan on improving efficiencies in a design or process, without compromising safety and/or increasing risk.
