Generally, when a major setup happens we react and try to fix it without due analysis, which leads to additional problems. Conduct a through root-cause analysis, even if that takes some time, keep the client's confidence, and have a consistent and transparent communication channel. It is also critical that people involved in the setup are not doing root cause as they could be biased, try and get an independent team, and provide a clear path forward. Try not to dwell deep into past issues but focus more on the next steps and a clear roadmap of correction and tracking
