Remember, before a brakdown happens-it will show manifestation of process instability. It is equally important to know why was the instability not given the proper attention.This is now the Defect Handling program should come into focus- is it robust, does it has leadership support, is it accessible to all.

In my experience, the first step would not be conducting a RCA but to stop the "bleeding". If the output or product that is being delivered to the customer is in risk of not complying with the expected quality, then the full delivery process should be stopped. RCA can be performed in parallel, but stopping the leak is more sensitive.

We have to do RCM and FMECA, have a complete list of all critical parts. Then we can work it out with the major suppliers for all critical parts readiness in their warehouses, especially for pricey parts; so we don't have to keep idle funds in our own warehouse.
