You've discovered product quality deviations. How will you effectively address them with your team?

Deviation management is a critical process ensuring product quality, safety, and adherence to regulatory requirements. I Will address it effectively identifying, investigating, and resolving deviations in a timely and compliant manner. The deviations must be analyzed and investigated with the support of a multifuncional team in order to identify the best solution. Without an efficient management system, deviations can quietly pile up, leading to a dangerous backlog that puts companies out of compliance with their own policies and even regulatory expectations.

First determine if it’s a noise or trend (one off case or repeat instance). Secondly it’s impact. Impact on regulatory requirements or patients safety address it immediately even if it’s a one off case. Third identifying root cause … at times it’s challenging….. but always remember…. If one can identify the root cause … your problem is solved because you know exactly what needs to be done.

Any investigation in regards to deviations aims to identify the causes of the deviation. If you correct the cause…!! Problem solved..! However its not always the case. Thorough investigation is required with investigation tools like fishbone, 5Why etc. Moreover-experience, process knowledge, product behaviour, material properties, clarity in instructions/ Procedures will help to deep dive. Operator interaction also gives the user point of view in identifying the cause. Once identified measures to correct and prevent the cause needs to be implemented with effectiveness monitoring. This will give a long term correction and quality in product.

Identify the root cause and communicate the issue clearly to your team. Collaborate to develop and implement corrective actions, ensuring everyone understands their role. Monitor the results closely and provide additional training if needed. Regularly review progress and adjust strategies to maintain high quality standards.

Product quality deviations should be tracked through statistical process control. It is an iterative process that continuously measures deviations. Use a simple philosophy such as Plan Do Check Act model. The Check says if our plan was effective and the Act is an opportunity to test something new. Repeat cycle till the stats confirm that we are back on track. The DO part can be made more effective using project management standards and the plan can be made more productive by using agile methods such as scrum methods having the customer in the room. The product is a living organism and lessons learned and training is important to cover those lessons learned and to better understand the nature of deviations and their impact.

Root cause analysis is the logical base for a CAP, but only if it’s carried out correctly. Many factories are doing it as tick-box exercise to present it to the customer, but it total disregard to whether it outlines the actual cause. The real RCA is never one person’s job. It must be carried out by intra-departmental team, where all possible reasons are listed, discussed, checked (!). A process of elimination then outlines the most probable cause. Often, it is a combination of causes. Then, a CAP can be created, with a purpose to prevent the issue from happening again. CAP should be a functional plan, not a docket filled in with cliches, and it should be verified through regular follow ups and adjustments until its effect is confirmed.

In that type of cases are very critical from a customer point of view, at that time arrange a combined meeting with operation and quality for analyses what happened there RCA etc, and most importantly customer what needed and how you fit your product on that immediately

Before conducting the investigation the multifunction team should come up with a investigation plan and identify right rootcause tool to use depending the case . The investigation plan enables the team to know the areas to review and gather information to come up with the probable root causes.

Create a corrective action plan with urgent actions to rectify deviations and stop recurrence after the root causes have been determined. Establish deadlines, delegate tasks, and set specific goals. Make sure everyone on your team is aware of their responsibilities for carrying out the plan and how they help to restore the quality of the final result. Review your progress often in order to stay on course and make the required corrections.

Sometimes corrective actions are very difficult to implement, especially at later stages of product lifecycle, when resources are very limited. It is crucial to properly balance between risk & benefit and define realistic goals. This will ensure trust within the team.

It is always important to remember that communication needs to travel in all directions. Encourage your team to not just communicate with you but with each other, as well. Each member of your team will have a unique perspective on any given issue. Always remember that everybody's input is valuable.

In order to handle variations in product quality, training is essential. It is imperative to guarantee that all team members receive comprehensive training following the implementation of corrective measures. This can be achieved through workshops, e-learning, or hands-on sessions. This averts more problems and gives the team the tools it needs to maintain quality standards. Additionally, ongoing training facilitates the onboarding of new hires and informs existing staff members about best practices.

Most of the times changes are not welcomed. At this stage I find it useful to explain through the customer eyes. Explain why this new step is crucial to ensure customer satisfaction and what will be the effect on final user.

In any process there will always be process or quality deviations. The questions are how much deviation and what action is required, if any. And if you have to? do a full blown quality investigation and corrective action planning rather than a few tweaks here and there, then you're already late. There are two main goals here - first and foremost is Prevention.?And if this goal can not be achieved, then early detection is second. Process Control and Quality Monitoring preferably with continuous and automated systems prevent significant quality deviations from arising, avoids surprises and labour intensive quality investigations.?
