To make sure that no information is missed during handover (handoff), easiest to use a standardized checklist for each patient. This patient care summary would include all possible fields of patient care & info from the previous shift. Do handovers in person, at start of incoming shift, to ensure direct communication & offer opportunity for questions. Update electronic health records before end of shift. To avoid info overload, especially with multiple patients in unit, highlight only new issues & important updates - like change of meds. Ensure no distractions during patient handover. Team leads should be knowledgeable about every patient in their unit and be the central point of reference for their team.
