Don't throw the baby out with the bathwater - do not lose an opportunity to learn when faced with a failure. After analysing the cause for failure - 1) share the lessons with team 2) put in practices to ensure such causes do not lead to failure in future. 3) Be cautious to avoid a BAND-AID process which can only address a temporary cause, and becomes an overhead in the long run.
