Be transparent during analysis of failure. Let team know what went wrong? why? when? whether there was alternative or mitigation? any Plan B, Plan C was identified? Which was the major reason? Try to avoid because of whom we failed (ideal situation) but there can be exceptions as well and need to discuss Accountability and Ownership in critical cases. Its easy to say success and failure are 2 sides of same coin i.e. efforts. But at the end of the day, success has many contributors and failure has noone to take responsibility. Organization can not run on catchy phrases or impressive quotes. So be honest and create failure analysis summary in detail followed by plan of action for future.
