In my experience ,majority of these kind of situations arise due to communication gap or misunderstanding between different stakeholders. Hence to mitigate the situation , one can do the following : 1. Take the inputs from all those involved and set up a proper timeline to establish the point of conflict. 2. If the delivery deadline is already crossed, then make all the possible efforts to make sure it is completed in the minimum possible time. 3. Make a good reputation with the client so that they are also at a understanding position and no goodwill is lost.
