Ask for candid feedback from stakeholders and challenge them with the 5 why method. I have seen many different issues, here are some examples: - the managers did not understand the importance of the forecast. - the managers were not trained and did not understand the financials. - the data was not easily accessible to do the forecast well. - the timeline was too short as they received monthly closing only minutes before they had to hand the forecast. All those issues can be addressed once you know the root cause of your problem
