Your client's resources are dwindling. How can you maintain continuity and quality of care for them?

Continuity of care and quality of care are important for improving patient outcomes and satisfaction. Some strategies for maintaining both include: Communication Ensure communication is effective and consistent between patients and their healthcare team. Involvement Engage patients in their care plans and involve their families and friends. Coordination Coordinate care with other providers and ensure safe transitions between facilities. Documentation Ensure patient information is properly documented in medical records and on transfer and referral paperwork.

Assess Financial Situation: Have an open and empathetic conversation with your client about their current financial situation. Understanding their exact needs and constraints will help you tailor a plan. - Sliding Scale Fees: Offer a sliding scale fee or adjust your rates based on their financial situation. This can help make therapy more affordable while still providing the necessary support. - Referral to Community Resources: Connect your client with community resources, such as low-cost or free counseling services, support groups, or social services that can provide additional support. - Use of Telehealth: If not already implemented, consider moving to telehealth sessions. This can reduce costs related to transportation and time.

Clients don’t often take a deep dive into their resources and they are surprised to learn about all of the programs and help that are available to them. For the case manager, it’s not very possible to make a plan for the client without knowing the clients’ situations.

Client don't care how much you know until, They know how you care. Clear communication. Advocate when they don't have a voice.

Case management assessments upon intake and ongoing as part of discharge planning to ensure clients have and maintain access to community resources. Assess for: - Past and current health conditions. - Service utilization. - Socioeconomic and financial status. - Health insurance plan benefits and coverage. - Home condition and safety. - Availability of prior services. - Physical, emotional, and cognitive functioning. - Psychosocial network and support system. Clients not only need weekly therapy but also intensive case management services based upon their individual needs to achieve, obtain, and sustain treatment goals.

I always find it helpful to have a list of local resources at the ready. Having established relationships within the community creates ease of access points for clients who may need services in a timely manner. As a nurse coordinator I am able to connect patients quickly because I understand the requirements and abilities of my local resources. Assuring patients I know the organization and have worked with them before builds instant trust and eases suffering these situations can bring.

To connect the client to the community can only help both entities going forward. Having a community on which the client can depend has the potential to create a lasting relationship that could at some point become symbiotic.

Being available to listen to a client at the right time and performing a needs assessment to assist in planning care is essential to delivering quality care. Having a open door schedule allows the client to obtain the right care at the right time by providing all resources needed including community care.

Maintaining meaningful relationships with the community and stakeholders is key to fulfil the objectives of the client using the resources which are not available.

Advocacy is so important especially as you walk alongside someone on one of the most difficult journeys they will make. Being able to understand your client’s wishes, concerns and fears can be critical to ensuring they receive quality care.

Being a strong advocate of client’s needs is most important for the case manager to succeed because only a strong follower or believer can be a strong leader.

When resources are tight, creative solutions can make a big difference. Adjusting fees on a sliding scale or offering pro-bono sessions helps clients afford care. Group therapy provides support cost-effectively, and directing clients to free or low-cost resources can complement their therapy. Flexible scheduling and community partnerships also enhance access and support. These approaches help maintain care quality even with limited resources.

For every vulnerable elder, the division on aging needs to be engaged, and applications for all possible support done as soon as possible. Applying for Medicaid should be step one.

Let’s face it, the spigot drips when it comes to resources for our clients. We get big grants, government funding, in-kind donations and it’s depleted in what seems to be seconds. As nonprofit professionals we have two choices: 1. Get defeated, tell the client sorry but there’s nothing left. OR… 2. While waiting for the next funding rainfall; stay positive, get creative, and look at everything as a resource - see what you can do with what you’ve got. Recently we ran out of heat relief supplies and needed to make cold packs for our clients to use for immediate heat relief in our triple digit temps. We looked around & started cutting up towels, sweaters, etc to drench in water to freeze in baggies. We make 200/day just by opening our minds!

Our lives are made up of a"series of daily decisions". Some are small with little impact, while others that do create a major change require a plan. We are always driven by circumstances & situations. As a person we are always trying to paint a picture of how we see our lives, I call this a "vision". So how we move financially, What we want to be educated in and what we want to "become" is made up of time and motivation. Without challenges we will not change. Why am I saying this?. Because, I have worked 47 years in my life and I can look back at my creative side that made my 2 careers successful. Moved out of my Mom's house in 1978. Saved money to move to an apartment, That was the start. I am sitting in my Beautiful home now sharing this.

I have come across clients that are difficult to provide care and services for. Either they are not accepting or not respecting the services provided. This has required some creative thinking. Often it is doing a lot of historical research into clients past. Gaining consent to contact family, friends and health professionals is the start and interviewing them to gain full perspective of client. Then we are able to understand better how to approach clients and influence their behaviour and conduct towards providers. Once plans are decided boundaries should be explored. Then the care plan is prepared and reviewed with the client. This shows respect and allows them to see the information provided to the people caring for them.

Let's face it, therapy is not intended to be a forever thing. a good therapist will regularly be helping their client implement rhythms into their lives outside of the therapy office to help create a sustainable long lasting plan for wellness.

From my experience In case management, clients are the first in line of accountability for any care effort to succeed. They must be encouraged to take care of themselves through Knowledge of prophylaxis, commitment to their own health and overall well being before the attention of the health care professional is requested.
