Clearly defining your scope and pre-planning will avoid inevitable scope creep. - Did I work with my client, team, vendors, SME's to define requirements appropriately? - Are roles and responsibilities clearly defined within the change control process? - Am I meeting frequently enough with my client to ensure a transparent process while mitigating risk and addressing any concerns?

To avoid or to minimise this, it is important to get a sign off from the sponsor at every stage of the project. Communication with key stakeholder/s & other stakeholders is key.

This plan must be communicated to stakeholders and project team in the begging of the project.

First of all, it is important to keep in mind that all project participants are responsible for quality. Secondly, do not forget the “Bermuda Triangle” of Project Management (Cost, Quality and Time): any aspect that is affected affects the others. If you live only in planning, time will be your nightmare. If you do not take lessons learned + expert consulting, you may have trouble closing. And if you constantly change scope objectives, then time + money will suffer.
