What are the most important reports for tracking project risks and issues?

Risk management has 4 major documents: 1. The Risk Management Plan describes roles, responsibilities, processes, tools, funding, and many other aspects. In cross-corporate project business with customers, contractors, etc. it describes how risk management is done across the organizations involved. It also includes a core decision: “Yes, we‘ll do risk management.” 2. In the Risk Register, you list all identified risks with data from analysis and log simple responses. 3. The Risk Response Plan describes how the project team will respond to significant risks, if the description exceeds what the Risk Register can cover. 4. The Risk Report escalates Risk Responses to higher echelons when the team is not authorized to respond to them.

The cornerstone for tracking project risks and issues lies in maintaining a comprehensive Risk Register. This document should encapsulate a detailed account of identified risks, their potential impact, and proposed mitigation strategies. Regularly updated and reviewed, the Risk Register serves as a dynamic tool to monitor risk evolution throughout the project lifecycle. A risk register is very important when managing risks. It's one of the first set of artefacts needed for a project as you start tracking risks from day 1. It's also important to review this document frequently with the teams, making sure the top risks are shared in periodic status updates

I keep it is simple in my projects and use a RAID log either in a excel document format or in a Jira dashboard. RAID log includes all the risks, assumptions, issues and dependencies of the project with appropriate risk mitigation startegies, assignees, due dates, etc. This artifact is only useful if it is used on daily/ weekly basis, is updated on regular basis and used by all counterparts of the project. I always share it with project stakeholders and show open items regularly (either on weekly report calls or other stakehokder meetings). Moreover, regular status reports and trend analysis can also provide a holistic view of project health.

So I've 6 on my list No. 1 is Mitigation Progress Report → This helps track progress on risk mitigation strategies for proactive management. No. 2 is Trend Analysis Report → Helps to identify patterns in risks and issues over time. No. 3 is the Stakeholder Communication Report → Document and analyze transparent communication on project challenges. No. 4 is Lessons Learned Report → You extract valuable insights from risk and issue management for continuous improvement. No. 5 is the Resource Impact Report → You evaluate the resource implications of identified risks and issues. This report helps prevent potential bottlenecks. The last one is the Cost-Benefit Analysis Report: Conduct a cost-benefit analysis of risk mitigation efforts.

An issue log is a project document that can be used to record and monitor all issues encountered within a given project, documenting relevant information regarding their type, description, priority, and status, as well as who raised them and who is responsible for their resolution, thereby helping a project manager to effectively track and manage those issues. Throughout a project life cycle, the log should be consistently updated as a result of monitoring and control activities, capturing any new issues that arise within a project or changes in currently recorded issues, and keeping in mind the continuous communications with the affected stakeholders for effective management and resolution of those issues that may affect the project.

I cannot stress the importance of an issue log enough. Not only does it create a historical document, but it also allows team members to work in harmony. Write out a brief description of the problem(s), possible solutions, actions taken, conversations surrounding the issue, potential risks, and ultimately what worked (and what didn't.) It takes a little extra time to do all of this, but you and your team will benefit from the information in future projects.

A risk matrix is a versatile tool in risk management that aids in the systematic evaluation and prioritization of risks. By visualizing risk information in a structured format, organizations and project teams can make more informed decisions and allocate resources wisely to mitigate potential threats and opportunities effectively.

A risk matrix is widely used and basically is a 3x3 (or 6x6 if you prefer, see below) grid with the axes being the probability and the impact of the risks. I have used a simple 3x3 matrix in all kinds of projects and it was always sufficient since it can be understood easily also by people who do not work on projects all the time. On one of my current projects, the program above was classified as "key program" of the organization and some kind of controlling authority requested the risk management must be improved. What they required was the program and all its projects switching from a 3x3 risk matrix to a 6x6 matrix. What a bullshit, the risk management has not improved by this measure! It just gives a pseudo certainty or security.

If they had a risk matrix, perhaps they could have maintained the beauty with a bit more ease. Our project is our own Hanging Garden, and we aim to make it flourish.

Again, risks can be registered through an issue log (keep it simple). Many resources guides you on how to do so. Once risks are Identified, matrix can really help to visualize and choose the best way to respond. Don't do it alone engage your team, they have ideas and expertise that will bring value to the table. Explain each of the categories to the team. Once discussed share the document with your stakeholders their feedback matter and if possible engage them with this process if not sharing is enough. Keep updating as the project progress and reflect with your team on that.

Include the proposed solution but make sure you update actions taken and the final resolution of the issue.

Also check archives for historical documents from similar past projects to assess risks. I am only starting to use these, but AI tools can help add insight to predict risks and outcomes. Collaboration software is also key to keep all stakeholders informed.

In my experience, the most important reports for tracking project risks and issues are those rarely discussed or prioritized. So, instead of focusing solely on formalized reports, consider these unconventional approaches: 1. Serendipity logs. ? Document positive outcomes from perceived risks to drive optimism. 2. Failure festivals. ? Encourage open discussions about past failures to promote continuous improvement. 3. Risk poetry. ? Express risks creatively through poetry to engage your team differently. 4. Anonymous risk confessions. ? Provide a platform for anonymous sharing of concerns to uncover hidden issues. 5. Gamified risk dashboards. ? In my experience across different verticals and climes, the real value additions to business is perhaps how fast I can help business close either the identified risks or raised project issues without halting the planned flow of tasks, deliverables or expected outcomes. In a multi country implementation years back, I was able to quickly identify the possible risks I might faced managing a diverse and multi cultural team. In my own perspective these are some of the most important thing to consider which most of us neglect - Communication Skills (Doesn't matter if a person is introvert or extrovert , it's very important to communicate with others when we are in a professional environment) -Microsoft Office ( What is the best software for Project Management ? Some might say Asana or others dedicated to this purpose , but in my opinion learning MS Office is much more important at first .) -Adapting to new skills ( There was a saying in Bhagwat Gita , that we all should be like water and able to take the shape of the container in which we are poured . We all need to adapt to new Technologies and skills be it AI for now or any other thing in future.)
