Risk culture can be further strengthened by learning from our mistakes in the organization and reporting risks, misses and near misses without any fear of any kind of blame or punishment. Often it has been observed that the organizations don't report all the risks. Even if majority of the risks have been reported even then near misses are not reported for fear of blame or being labeled unprofessional and because of fear of punishment. Such a practice makes the operational risk exercise of gathering, evaluating and mitigating the risks just a futile exercise without much value.
