What are the most effective ways to report incidents in power engineering?

Organizations should have a clear procedure for reporting incidents, and be consistent in applying discipline to employees who do not follow these procedures. Organizations should also apply clear SOP and signage so that employees know what to do in the case of an incident.

Employees should be trained on what to do during and after an incident. Ideally a team should be educated on how to avoid an incident and proactively trained on hazard communication and all pertinent safety practices.

Uninhibited access to all reports for site management, especially those responsible for a site out of hours, helps to create a safer working environment for all, and increases awareness of issues on the site for which they are responsible.

Access to this information should be made readily available to the entire organization and if possible published to be shared within the relevant industry. The information should also be properly archived to be used for future reference within the company and as learning material for new employees as the organization grows.

Companies should have a lesson learned meeting or similiar type of meeting after incidents, so that employees and staff can discuss what went wrong and how to avoid it in the future. Incidents should be communicated to employees whenever possible so that employees can learn from the mistakes of others.

The value gained from promoting collective responsibility cannot be over emphasized when fostering a proper safety culture. It is the responsibility of each employee to ensure that not only they operate safely at a personal level but also to correct others who pose a potential risk to themselves and others by unsafe acts.

Preventing reoccurrence of an environmental, health, safety or quality issue is the key output from a reporting system. If learning is not happening, and repeat events are observed regularly, then the actions placed need to be reviewed and better actions placed in the future. Resource needs to be available to service this.

There is no task that is so important that it cannot be done safely. Employees should always be reminded of this and reminded that safety is everyone's responsibility.

The RCA should be completely unbiased (if required approved by learned neutral 3rd party equipped with all relevant measured/ recorded data) based on criticality & risk level of the incidence.

# Report must be easily accessible for anyone even for someone who joined new the organization newly. # " Near-miss" : Near miss reports must be recorded and appreciation rewards to be promoted for reporting near miss. # Most importantly every individual at the work place to be trained and reminded frequently to have empathy towards fellow human and their family, so that they perform their duty with quality and accountability in order to ovoid even a minor ignorance while performing their duty which can create an incident.
