What are the most effective ways to interpret language in healthcare?

Professionals must be trained on working with interpreters. Some golden rules: 1) Speak directly to the patient, maintaining appropriate eye contact, 2) Use the patient's name to build rapport and trust, 3) Do not use expressions such as 'Can you tell him/her' and address the interpreter, 4) The patient should always be addressed directly, 5) Provide context - 'Today's appointment is to discuss your test results/review your treatment', 6) Ask only one question at a time, 7) Leave enough time for the patient to answer question, 8) Allow an opportunity for the patient to ask questions, 9) Provide a summary for the patient at the end so they understand what the appointment was about, 10) Use simple language and avoid jargon.

Rule No 1: do not allow children who are relatives of health care users interpret for those relatives. It is unethical to put children in such positions and also removes work from qualified and professional interpreters. Rule No 2: pay professional interpreters properly. Rule No 3: do not use poorly qualified or unqualified interpreters. Rule No 4: do not allow agencies to take over the game - they need full regulation to prevent scurrilous behaviour.

The modes of interpretation in healthcare can be either consecutive or simultaneous depending on the type of meeting, patient and provider preference. In healthcare, in-person interpretation is preferred by the majority of patients and providers, as it allows for more personal and nuanced communication, as well as visual cues, gestures, and body language.

In healthcare, the most effective language interpretation involves a combination of linguistic proficiency and cultural sensitivity. Skilled interpreters ensure accurate and empathetic communication between patients and healthcare providers. Leveraging technology, such as AI, can also enhance accessibility and bridge language gaps, making healthcare more inclusive and responsive to diverse patient needs. Effective interpretation goes beyond words; it's about providing quality care to all, regardless of their language or background.

In a healthcare setting, it is even more important to not just familiarise yourself with the medical terminology and the ancillary jargon but also get the patient's concerns across clearly. This requires active empathetic listening so that levels of discomfort and/or pain can be communicated effectively so that the healthcare provider can take the necessary next steps. It takes a nuanced approach which requires an interpreter to work efficiently in a chaotic and highly stressful environment (more so than the usual high pressure situations that interpreters face in their job).

I would make a distinction between long consecutive and short consecutive. What was described is true in many cases of long consecutive, which can last from several to many minutes. Short consecutive consists of rapid interchanges of dialogue, often lasting no longer than a minute or less. It’s used in court, healthcare or other public service settings. The type of discourse may be narrative, but is more often disjointed, and with frequent hesitations, regressions, and other linguistic challenges. In settings where short consecutive is used, often the person in need of interpretation is trying to tell a story in some detail while the person with whom they’re talking is typically asking questions and expecting short, concise responses.

Interpretation can also be classified according to the level of interaction, i.e. Onsite or remote. The use of remote interpreration has been the method of choice since the pandemic started where everyone had to stay home. This method has proven to be convenient and practitioners together with patients had been continuously applying it for their language needs. One common example where remote interpretation offers a great advantage is for mental health patients, wherein consultation would require a lot of listening and understanding, but not necessarily focusing on the physical condition, unlike in other health ailments

We need accuracy when focused on the targeted language. We don’t loose the core value of the domain subsequently the main idea is the key.

Interpreters need to be proficient in various interpreting styles and be able to switch between them on the fly. It is very common for when using the consecutive interpreting style for the conversation to start to flow more freely to the point where the speakers aren't pausing and you need to seamlessly switch to a simultaneous interpreting mode. When interpreting over the phone or virtually, note taking is a MUST especially since the interpreting mode is consecutive. Sometimes speakers will say over 50 words that you have to interpret accurately and without notes there is a risk of saying the wrong words and even leaving out crucial information. These are only a few possible scenarios.

Effective healthcare interpretation ensures clear communication between patients and providers. Interpreters must master medical terminology and understand cultural contexts, while upholding confidentiality and neutrality. Active listening and empathy are crucial for addressing emotional situations. Continuous learning and collaboration with healthcare teams improve accuracy and effectiveness. This blend of technical expertise, cultural awareness, and interpersonal skills is essential for enhancing patient care and outcomes.

To gain a bit of confidence with the client and allow the session to be the most efficient: communicate to the client your role and goal in the interaction. For dialect request and explain to client to take their time when responding. For example if a Central American Spanish interpreter has a Cuban, Puerto Rican, or Dominican client the speed and dialect may vary.

One important skill to master when interpreting, is to remain objective or be neutral. Although you serve as the advocate for the patient, making them feel comfortable with their healthcare provider, you would never want to unintentionally add bias to your interpretation. What if something doesn't go as well as expected. Maybe the patient has become frustrated, or perhaps the doctor or personnel speaks frankly. Although you're familiar with the patient, you would always interpret accurately and maintain the tone of the message source. If you need to state your position as the interpreter, do so, out of respect for the message being conveyed.

*Listen carefully to both parties. *Interrupt them if you don't understand anything. Intervention is better than confusion. * Convey the message accurately.

It is unlike in translation, where you have time to think about the choice of words and make changes. Interpretation is more dynamic and requires a quick mind.

When I used to work at a local healthcare system in the Bay Area, I observed that interpretation services can be incredibly complex and inconvenient, particularly when it comes to rare languages. In Oakland, specifically, there has been a noticeable increase in the number of Mayan Mam Speakers, who speak one of the many distinct and unique Mayan languages that are still actively used today. This presents significant challenges in finding the appropriate interpreter, especially in emergency situations, where the ability to effectively communicate with desperate and anxious family members and patient is crucial. It's important to note that the Mayan Mam language itself consists of various dialects, each with its own distinct characteristics.

Re the last phrsse, I may not agree about "error prone", in spite of emotional stress, based on personal experience with witnesses on the stand, where, yes, empathy emotions can be difficult to manage -specially if it is quite apparent that a defendant is being unfairly prosecuted or a witness is heavily aggrieved/emotional. As to terminology, Mexicans refer to a child's anus as "colita", which translates as "little tail", for the benefit of anyone who may not have encountered this.

Languages aren't just about words; they carry cultural meanings, idioms, and expressions. Misunderstanding these can lead to misinterpretation and even offense.

One thing I have found helpful is to have confidence coupled with humility: You need to assure the 2 parties that you are able and willing to help during the cession but also you should remember that overconfidence also may be dangerous in the healthcare setting. If you don’t know something, please request for explanation. An Interpreter is not a medical doctor, there are things we know and other things that we don’t know. When you are humble enough, providers like it and give you details on something. Our goal should be to make both parties happy at the end of the cession.

J’ai exercé comme médecin en France où j’ai appris la médecine dans les années 70; puis exercé en milieu universitaire; puis exercé en Canada anglophone toujours en h?pital universitaire à partir de 2006. Je suis retraité et travaille comme interprète principalement dans le milieu de la santé. L’enseignement de la médecine a beaucoup changé, les médecins maintenant parlent plus simplement qu’auparavant, ce qui rend plus simple la compréhension de ce qu’ils disent. Ils ne disent plus ??dyspnée?? ou ??dyspnea?? mais vont dire ??difficulté à respirer??, ??difficulty breathing??. Les exemples de cette simplification sont multiples, ils suppriment la nécessité d’interpréter le jargon/argot médical dans ??le parler de tout les jours??.

Putting LEP's needs first is my go to strategy when facing difficult situations. Patients are not always right, but they are always vulnerable. They need to trust not only their healthcare provider, but the person facilitating the communication between them. Interpreter and provider alike must understand that and work toward assuring the LEPs that their wellbeing is the goal in the whole interaction.

Empathy is for sure one of the best tools when working as an interpreter and at the same time, an interpreter should not take sides.
