What are the most effective ways to communicate with terminally ill patients?

It's not easy at all, but practice empathy. Provide clear information and listen actively. Allow space for emotional expression. I'll give you some examples: "I'm here for you. How are you feeling today? Is there anything specific you'd like to discuss or share?" "I understand that discussing treatment options can be overwhelming. Let's take this step by step. What are your thoughts and concerns? Your input is crucial, and together, we can make decisions that align with your values and wishes." "I can't fully understand what you're going through, but I want you to know that I am here to support you. If there's anything on your mind or if you need someone to talk to, I'm available."

In my communication with terminally ill patients, and really anyone, I strive to connect authentically from my heart. By seeing the light and divinity in each individual, I approach conversations with love and affection, often holding their hand or placing mine on their forehead. We are all alike, and communication should be rooted in love, respect, and honor, reflecting our true essence. Breaking down the walls that separate us allows for a natural flow of gratitude and appreciation. Let's start with the obvious: Tell the truth about what's happening and what's to come. It might be hard to hear or understand, but knowing what behaviors or symptoms to look for can help patients and their caregivers identify and address behaviors that indicate an acute need. For example, the common cold can lead to exhaustion and not drinking enough water. In a matter of 2-3 days, a UTI can cause delirium and dizziness, leading to a fall. Most falls happen at night on the way to the bathroom, accelerating the decline. Instead, tell people BEFORE they are in the throes, when they and their caregivers can plan and act on the guidance.

Be honest and transparent: While it is important to be sensitive, it is equally crucial to be honest with patients about their condition. 4. It’s an important balance when communicating options. I’ve seen practitioners state facts, percentages etc and completely overwhelm the patient. Being realistic is key but also being compassionate and empathetic. Being able to meet the patient and their family where they are at. Most of what is said in a meeting such as this is not heard. Making sure they have someone with them to write things down or if not available provide them with something written. It’s so hard when you are hearing all the options to process, understand and apply it to yourself. Completely agree with their right to refuse. Having conversations earlier about palliative care helps with later conversations.

Palliative care physicians have to do 2things well. 1. Develop a individualized prognosis for every patient to include multiple factors: What I call the "D's": Disease progression, Disability, Dollars, and Death. For many patients it is not all about if they are going to die, its about how there going to live and how they will live, along with how much is it going to cost. 2. The 2nd and most important is communicate that prognosis in a compassionate and empathetic manner. The first is the science the last is an art and regrettably where so many of us fail.

Empathy is the ability to understand and share the feelings of another person. It’s a basic human trait that allows us to connect with others, build relationships, and feel compassion. Empathy is essential for creating that much-needed connection between doctor and patient. At its core, empathy allows us to put ourselves in the shoes of another person and understand their perspective. It can help us be more understanding and compassionate towards them, no matter what situation we’re in. This heightened level of understanding helps us to better relate to one another and can even bring about a greater sense of calm within ourselves.
