Document which covers all mentioned aspects is usually called Testing Strategy. Defining a mutually agreed Test Strategy is very important as it determines how testing will be performed and completed in the project. Before testing starts, all project members must agree and align to the coordinated approach. If you are implementing a well known ERP, it most probably will come with implementation methodology and guidelines and I am more than confident, it will have a Testing Strategy. So first - check the guidelines, so you don't need to start from scratch. E.g for SAP Implementations, Testing guidelines and Test Strategy template can be found as part of SAP Activate Methodology roadmap accelerators, that can be used as a reference.

The test cases should include all the possible scenarios for a function. This should be decided on the basis of the feedback from users, stakeholders and business process owners. This test layer should act as a predecessor to the UAT phase.
