Data quality issues will almost always be apparent in data ingestion activities as there's only so much validation that can be at the point of data entry. In my experience of implementing a data warehouse or similar ETL application is to handle data quality issues within transformation Stored Procedures utilising T-SQL. For example, postcode validation can be done through a simple function using regex. Any postcodes that don't fit this pattern can be written to a data error table (FactDataError might be a good name) and subsequently raised on a DQ Dashboard for Data Stewards to analyse. You could then take this further by writing another DQ 'rule' to check for postcode accuracy with a lookup to POLAR data as an example.

Schema mismatch is almost inevitable, particularly with data migration activities. A useful tool when migrating data (or ingesting data with different schemas) is the Entity Relationship Diagram. This is essentially a high-level link between each of the entities (or tables) within a RDBMS and will assist in mapping attributes or fields from one schema to another. Once the mappings (often referred to as requirements) are complete then T-SQL can once again be utilised to handle the transformations as outlined in the mapping document. Bear in mind these mappings could be 1-to-1, 1-to-many, or many-to-1 depending on the exact schema and relationship between entities.
