From the perspective of the person being referred to (non-medical HomeCare), we really need as much information about the patient as possible: health conditions, recent incidents that brought about the hospitalization and rehab, treatments, and results, expectations for treatment and follow up care after discharge, what to expect and monitor. Our goal is to provide a service that will help the patient to have positive outcomes and improved quality of life. And to keep them from needing another trip to the hospital.

I have found the most success by reaching out to the provider first to ensure that they have availability, etc. and I prefer that colleagues do the same before sending a referral my way. Maybe it is more just a courtesy but I always feel disappointed when a patient says that Dr. X referred them to me and I have to tell them I have no openings. This type of situation reinforces the need for relationship-building in our quest to navigate through the rough waters we call healthcare.

Also consider if your client prefers a male or female physician or if the physician needs to have appointment availability beyond traditional hours.

You'd be surprised by how many patients are referred for a specific problem such as a joint pain, and the referring provider or physician did not even have any imaging done yet. This both delays care for the patient and puts unnecessary redundancies on the specialist.
