The first step in any problem solving methodology in any organization for that matter is to understand what the problem or hindrance is. The first indication of a problem with respect to a layman would be a obstruction or lack of result. Defining a problem from here basically means going into the depths of a problem and describing it in vast expanse. A project charter which acts as a preamble for any problem solving mission contains all the tools that would help in defining a problem. This includes the problem statement, business case, purpose of the project, goals/deliverables, scope, stakeholders, schedules and timelines, etc. Hence a well structured project charter would define the problem comprehensively.

To define the problem statement clearly is very important part of any problem solving technique and without the clear statement we may end up addressing the wrong issue/problem. I find it's important to listen closely with unbiased intent and gain all information pertaining to the issue. One thing I have found helpful is to encourage first line staff doing the work to come up with solutions while trying the new process. Additionally, requesting frequent feedback from first line staff regarding the implementation process. This allows them to take ownership of their duties and work on their collaborative skills. It also allows leadership to address any issues or concerns immediately.

As they say, ideas are pawns while execution is King. After a reasonable initial assessment, trying different calculated solutions will greatly benefit the entity.

Initially we need to identify why the gap is it, I mean if everything is already pre-planned things are in action and after that we find some Gap's like coordination issues, lack of proper implementation, skills lack. then we should overcome these gaps otherwise, if we are not aware about gaps then we can never fullfill them. ??

As Assistant Manager of the Gap 1987-1990 & (responsible for scheduling). Employees complained for more hours, or certain days. I had an idea that made scheduling easier, "Who gets what hours?" My idea, they could earn hours they wanted. I had a small paper for each employee, for each shift, they could earn 1-3 points in 3 areas, #1-Did they dress "Gap like"? #2-UPTs sold? (Units per Transaction) #3-Did they reach their sales goal? I divided each employees' points earned/hours worked each pay period. Wahla!! They earned the hours they wanted to work. Scheduling was much easier, & no complaints. I left Gap in1990, I was recruited by Nordstrom. My Gap Manager took my idea to corporate, and it was implemented into Gap practices. (she said)

Yes, it is true to identify the Gap, problem or process lack and then try to find out, why these gaps are because of some person or technique or technology. And then try to work on it, in terms of process, innovation or technique, or training.

This is all great textbook information. But it’s been echoed in government for decades. With little to no real change. There are underlying problems that go beyond these steps. We need young agile and zealous professionals that are willing to take risks, make necessary changes, and think differently. Otherwise these steps in this article will keep getting repeated in articles, trainings, and conferences - as vain words with little change

I'm in agreement with what has been posted so far, but recommend reducing the scope of the project from the get go. One of the problems we often run into when we set up the project or contract is to assume we are going to build the big solution immediately instead of testing the idea for viability or usefulness. Define an MVP first, do a challenge, do a phase, do something to confirm the investment is worthwhile. Once we do that, our decision to move forward or not makes us strong stewards of tax-payer dollars and takes the theoretical into the practical. In addition, I highly agree with Will's comment. We need the risk takers to be strong enough to end a project when investment isn't worthwhile.

Problem identification is not a simple process. I am often asked to provide education to an entire hospital service. I always propose a gap analysis first. A gap analysis identifie the gap between current and desired state. A root cause analysis defines why those gaps exist, you have to know the exact/specific gap to accuratly analyze the root cause. Find the gap, identify the root cause, then choose an approach and implement a solution. The gap is often not at the level of a service line, but is instead a smaller group or individual who are at the "root" of the problem. Be certain you have a systemic/endemic gap or problem and not a compliance problem. Many of the problems I see in Clinical Nursing Education are complance issues.

So, Gap was not a government agency. But there was a problem, I figured a way to fix my problem with scheduling, it helped moral, sales and helped me with scheduling employees. We kept good employees happy by hearing them and coming up with a solution. It was just a simple idea. But it helped employees feel heard.
