Adding to the "Assess Needs" step, don't only examine the constraints of the legacy system, clearly identify any unique functions the legacy system provides that are a priority for the user AND cannot be easily implemented in the new technology. I know this must be balanced with legacy components that have been "sunsetted" or that expose the organization to vulnerabilities and with users who are resisting change. This is also a good time to assess workflows and processes that can be automated to create a better user experience.
