Document should give an outline of your issue and state how & where it is occurred . Once the explanation is demonstrated. Check for grammatical mistakes, tenses and review it by your senior peers.

Consider the software development documentation as a book and there might not be a case to make it the best in one shot. What I have tried is by doing a walkthrough of a project with the documentation and treating it as a manual to understand the code and technology. I believe we should think about how well the visualizations help everyone understand, even people who aren’t technical. We should arrange the visualizations so they follow the order of how things work in the software. We should also show who does what (roles & accessibility) & make sure it's easy for everyone to use. This can really help everyone understand,whether they're technical or not. Adding interactive parts to the visualizations can also make it easier to understand. It lets people get involved & see how the software works in a more active way. We should have separate visualizations for three main things: how the product looks (UI), how the technical stuff works(technical architecture), & how it affects the business(impact analysis)

Sadly it's a fact (generally) that while there is always time to pore over code (it's self-documenting, right?) there's never time for documentation. There needs to be a commitment from management to allocate adequate time for documentation in all development or maintenance tasks. Without that, documentation will always be the red-haired stepchild of software engineering. As for improving existing documentation, I think we need to borrow a practice from the construction industry. If your 1980 house burns down, when it is rebuilt it is to 2020 standards. Every time an object is touched, for enhancement or bug fix, part of the task must be to bring documentation up to current standards. This must be enforced and time must be scheduled for it.

Documentation should not be static. Based on the feedback received and the evolution of the software itself, continuously update and improve the documentation. Regular reviews can ensure the content remains relevant, accurate, and user-friendly. Set a schedule for periodic updates and assign responsibility for this task to ensure it gets done.

As software adapts - it's documentation should too. Be kind to others and your future self. Its a good habit to form and carry through your journey as an engineer ???? It can make all the difference for a new team member growing their understanding and becoming productive. One approach that can work well is to store your documentation alongside your source code in markdown files. This doesn't have to be a single .md file in the root of your repository - have that document be a summary with links to other .md files which you can store in ./docs in your repo (store diagrams in there too - link to them in the documents). If you do this - documentation becomes part of your PR process and can't be neglected if your reviewers care too.

This is especially relevant to smaller documents, decision records, etc. where the requirements, options, decisions were noted, but they don't make sense when reading a few months later, because the contexts has changed.

Since the documentation gets updated regularly, it is important to maintain the version number & date of the documents, to keep a track of all the updates happening and the user involved. Also it needs to be ensured that the readers are accessing the latest version of the document
