What do you do if your Quality Management process encounters a major issue?

Identifying/defining the problem is very critical step. Without which the team will focus on unnecessary causes. Use data to define the problem, which should include Pareto, trend analysis and statistics analysis. The more time spent analyzing and defining the problem, the shorter the time it takes to solve the problem.

When you encounter a major issue, the first step is to identify the root cause and how it is impacting your product quality and the process flow. After this, you perform the RCA and accordingly implement the CAPA. Monitoring of the same is must. This will not only help in maintaining the process but also improve the product quality.

Once the rootcause of the problem is identified, first we have to focus to correct the running production. Then,the defective production should be quarantined.Post that viable rectification options should be worked out to salvadge the maximum products.

Identify the Issue: Clearly define what went wrong and its impact on quality standards. Containment: Act swiftly to prevent the issue from spreading further and causing more harm. Root Cause Analysis: Investigate deeply to understand why the issue occurred in the first place. Corrective Action: Implement solutions to address the root cause and prevent recurrence. Preventive Action: Put measures in place to avoid similar issues in the future. Continuous Improvement: Regularly review and refine your Quality Management process to enhance effectiveness.

Clearly define and identify the issue is the first step to solve and process major issues. The team should gather all the data, information and stakeholder involved in the issue. One tool that can help in the company is Root-cause analysis a process used to identify the underlying root cause of a problem on a particular non conformity, defects and issue. The purpose of RCA is to address issues and eliminate the source.

Stop the production, Hold the products or Notify the customers, these are the process we can do if we encounter and identify the impact to prevent the problem from spreading and worsening. It is recommended to have a documented information about Control of Non-conformity or Non-compliance process, products and services.

1. Hold all the products which is produced on that perticular issue 2. Informed customers to hold the products which is produced on that day 3, check all the Hold products 100% to identify the issue

It also involves assessing the impact: It involves evaluating the severity of the issue and understanding its implications for product/service quality, customer satisfaction, regulatory compliance, and overall business performance. This assessment helps prioritize resources and determine the urgency of corrective actions.

Containing the impact of adverse situations is crucial for mitigating their negative effects on an organization. This phase involves implementing measures to limit the spread of the problem and minimize its scope. Acting swiftly and effectively, coordinating efforts and resources to contain the situation, is paramount. Additionally, clear and transparent communication with all stakeholders is necessary to maintain trust and calm. Containing the impact not only protects the organization's reputation but also lays the groundwork for recovery and learning. In summary, it's a decisive action that demonstrates an organization's ability to manage crises and maintain operational stability.

Understand any quality issue small or big report first. Raise a deviation. Let cross functional teams know. Come out with an open discussion of what , when, where and how it happened. For me, "who" is the last to be examined. Trust all are determined to do good. Effective communication is the key. Look for process, procedural, communication gaps. Fix through corrective and preventive action. Do conduct follow up reviews for effectiveness. If required strengthen QMS. See any one who authorised to do that activity will not repeat that inappropriate act. Support quality team without prejudice. Quality is everything. Conscious effort must reach every person in the organisation. Conduct shop floor training frequently. Record, repeat, reassure.

Identify and analyse the causes using best quality management tools like ishikawa/fish bone/cause-effect, why-why, FMEA etc., taking immediate corrective measures and recommend appropriate corrective and preventive measures to fix the issue. Continuous monitoring and check the effectiveness of CAPA is a permanent solution to prevent the reoccurrence and maintaining stable QMS system.

Esta tratativa como já aconteceu será corretiva, mas é importante identificar as causas para realizar as análises necessárias para que n?o ocorra novamente. Existem várias ferramentas da Qualidade, tais como: Brainstorming, 5 Porquês Diagrama de Pareto Diagrama de Ishikawa PDCA, entre outras, que nos auxiliam neste processo de análise, portanto precisamos agir na preven??o, utilizando metodologias diagnósticas para conseguir manter uma boa gest?o de resultados.

Eg: Implementing corrective actions It involves executing the action plan according to the defined timeline. Effective communication and collaboration among team members are essential during this phase to ensure that actions are carried out effectively and efficiently.

Implementing changes is a critical step in any improvement process. It involves carrying out the strategies and solutions designed to address identified problems. This stage requires careful planning, effective communication, and organizational commitment. It's essential to have strong leadership guiding and supporting the implementation of changes. Additionally, constant monitoring is needed to assess progress and make adjustments if necessary. Successfully implementing changes comes with challenges but also offers opportunities for growth and innovation. Ultimately, it's a continuous process that drives the evolution and long-term success of an organization.

When implementing changes, ensure that you start on a small-scale to best pilot changes without risking large projects in the process. Be sure that those implementing the changes are on board and trust those with the plan to ensure that it is effectively executed. Likewise be willing to listen those executing the changes as they are the first ones to indicate something may be going not according to plan. Monitor the process and scale. Once the objectives are being fulfilled and expectations are met, expand the process out to fill out the all of the processes needing attention. An overall steady pace should be taken when implementing changes. If it is rushed you may miss negative indicators. If it is slow then there could be high costs.

In my experience effectiveness of corrective actions always comes with opportunity for improvement. Effectiveness of corrective action must be consider as either in KPI or objective. Due to this it will help in success of effectiveness as well as reinforce the quality management system in organisation.

Well explained. One thing I would like to add, is lesson learned - compilation, propagation and updation of lessons learned. Also amend the QMS Risk register accordingly.

In the business we say a mistake is not a mistake if we have not shipped it. Sometimes eating the cost of the mistake is cheaper than exposing your company to legal action or recalls. Learn from that and carry out CAPA exercise to ensure there are no reoccurrences.

Sometimes the only option for a business is a hard reset. Start over. Remember what made you… you. Get rid of the fluff and get back the basics that made you stand out to begin with. With that, you’ll start to challenge stale convention and begin to grow (again).

Keep the risk analysis always updated, helps a lot. So can prevent some events that may affect your Quality Management System.
