Monitoring tools may fail for a number of reasons, bad network connectivity, a second helping, but incorrect hand applying a wrong configuration. The key is knowing what your tool can do, report and show, then, learning all dependencies. Fix anything out of the ordinary. quick example: one time our SW SEM failed to collect logs. point was, someone copied the VM to a different host, but in the process changed its IP address. Was quick to resolve when we found out.
