The only time that you can effect change on a project that is going south is to address as early in the bell curve as possible. We all know that if we are at the peak of the curve and then try and address problems, it’s a loss leader. Transparency, honesty, understanding and culture can help when difficulties or challenges arise. There may not be an answer or strategy, but getting all stake holders in the same boat makes a huge difference.

Sometimes we come up with root causes and decide to change our process to overcome them. Sometimes its a recognition that this is a one off event, and that our process or decisions are normally adequate. we need to be sure and not change for the sake of change, but decide if this occurrence was unique and the rest of the time the bad decision would not have been made

Sometimes its ok for the error to occur. Making a bad decision at least allows the next step to occur. Sometimes its easier to make a bad decision and have it fixed later, then be paralyzed with indecision. Ive had workers get lost in evaluating costs between A and B. The consequence of indecision was worse than repairing the outcome of either A or B.

Last and not the least, design and implement a couple of recovery plans, to recover the losses incurred...if first plan flops the next plan shall have to work...
