One thing I have found helpful is to set clear goals and expectations. Clearly communicating our expectations and goals to our team members, help them understand what they need to achieve and it provides guidance on how to reach those objectives. We should revisit these goals regularly and provide constructive feedback to support our team members growth and development.

Because coaching and mentoring skills are such critical skills, leaders need to be careful as to how they employ them. If employed without a plan such that the discussions come across as haphazard, lazy, and without direction, the "leaders" are GUARANTEED to do more damage than good. As Manmohan already stated, practicing these skill consistently is necessary for making improvements. However, its also critical that the practice occurs after first learning some basic frameworks for what constitutes good mentoring and coaching, AND what's the difference between the two. It's like learning to play the trumpet. I can "practice" six hours a day blowing into a trumpet, but if I don't know how to position my lips & breathe, I will only make noise.
