Try to give feedback on (as low as reasonable practicable) ALARP approach with practical examples that caused harm in past experiences. It should be personal so workers could understand consequences. Emotionally encourage workers and give them impression that safety feedback is for their own benefit. Strengthen HSE policies and enforce penalties by prioritizing human life on everything else.

Audit the management team. If they are not practicing their own policies & procedures, this would not help the safety culture or any feedback on safety issues. I can attest, I have done this and when I asked to see their safety board the CAO asked a worker where it was!! Not a great example of leadership.
